It is late, your child is hot and miserable, and the bottle in your hand has an age chart on the back. That chart is a convenience, not a measurement. How much medicine a child needs is governed by how much child there is — body weight, because that is what the drug distributes through. The age bands on the carton are a stand-in for an average child of that age, and most real children are not average.
That is why paediatricians and pharmacists work in milligrams per kilogram rather than birthdays. Here is how to apply that principle to the bottle on your counter — and where the real danger sits, which is almost never the arithmetic. Why weight is the accurate basis and age is only a proxy
A solidly built two-year-old and a slight four-year-old can weigh exactly the same. The age chart hands them different doses; their bodies want the same one. A heavy child dosed by age tends to be under-dosed, the fever barely shifts, and the parent is tempted to re-dose early — which is how interval errors start. A light child dosed by age is pushed toward the top of the range for their actual size.
Weight-based dosing removes that guesswork. The concentration on the bottle is the most dangerous variable
Most home-dosing mistakes are recoverable. Getting the concentration wrong is not, because it silently multiplies every millilitre you pour.
The dose a child needs is a number of milligrams. What you measure is a volume in millilitres. The bridge between them is the concentration printed on the label — how many milligrams sit in each measure of liquid. The two standard US children's liquids are acetaminophen at 160 mg/5 mL and ibuprofen at 100 mg/5 mL, and those are what the dosage reference is built around. If your bottle says something different, every millilitre figure from any chart, app or calculator is wrong for that bottle.
This is not theoretical. Infant drops were historically more concentrated than children's suspension, so the same volume delivered a very different amount of drug — a confusion serious enough that US infant acetaminophen was standardised to 160 mg/5 mL in 2011. Older stock and imported products can still differ. Re-read the concentration on every new bottle, and if it does not match what you are calculating from, stop and ask the pharmacist.
Use the syringe in the box — never a kitchen spoon
Household teaspoons are not measuring instruments. Spoons pulled from an ordinary drawer span roughly 2.5–7 mL, so one "teaspoon" can be nearly triple another in the same kitchen. Dose in millilitres using the oral syringe or cup that came with the product, since those are matched to that product's markings. Fill it on a flat surface and read it at eye level. If the syringe is lost, do not improvise — a pharmacist will give you a proper one, usually free.
Intervals, daily maximums, and why the ceilings matter
Each medicine has a minimum spacing between doses and a hard ceiling per day. The dosage reference gives acetaminophen every 4–6 hours, maximum 5 doses in 24 hours, and ibuprofen every 6–8 hours, maximum 4 doses in 24 hours. Confirm both against your own label.
Acetaminophen is processed by the liver, and the margin between a therapeutic day and a harmful one is narrower than most people assume — injury can occur at surprisingly low multiples of the correct daily total. The classic accidental overdose is not a mis-measured syringe; it is stacking. Acetaminophen hides inside many combination cold and flu products, so adding "just some plain Tylenol" on top can quietly double the day's intake. Ibuprofen is harder on the stomach lining and the kidneys. Give it with food or milk where you can. It is a poor choice when a child is vomiting, refusing fluids or already dehydrated — exactly when the kidneys least want an anti-inflammatory. It also carries a firm age floor: the dosage reference sets ibuprofen at 6 months and older, with no exceptions at home.
Either way, write the dose down — time and millilitres, in a notes app or taped to the bottle. Two caregivers and no shared log is the most common double-dose scenario there is. If you suspect an overdose, treat it as urgent even if your child seems fine: with acetaminophen especially, symptoms can lag the harm by hours. Call poison control or your emergency number.
When not to self-treat at all
Fever is part of how the immune system works, not the enemy, and the goal is a more comfortable child rather than a particular number on the thermometer. Some situations call for a clinician rather than a dose:
- Any fever in a baby under 3 months old. That is a same-day medical evaluation, not a dosing question — fever that young is the thing to investigate, not the symptom to mask.
- Below the age floors. Acetaminophen under 12 weeks needs a doctor's direction first; ibuprofen under 6 months is off the table at home.
- A fever above 104°F that does not respond to an appropriate dose.
- Warning signs alongside the fever: stiff neck, unusual drowsiness or difficulty waking, difficulty breathing, a rash appearing with the fever, or dehydration — dry mouth, no tears, markedly fewer wet nappies.
- A fever running beyond 2–3 days, or a child getting worse rather than better.
Trust your instinct. Paediatric practices field fever and dosing calls all day and would rather answer the question than manage the error.
The "never" list
- Never give aspirin to a child for a fever or viral illness, because of the risk of Reye's syndrome. Check labels for salicylates in remedies you might not think of as aspirin.
- Never use adult formulations for a small child. Children approach adult dosing only around the 95–100 lb mark for many products, and chewables and junior strengths exist to bridge the gap — ask a pharmacist rather than splitting an adult tablet.
- Never double up on products sharing an active ingredient. One acetaminophen source at a time, counted across everything.
- Never carry a millilitre figure between bottles without re-checking the concentration.

What if my child vomits the dose straight back up?
If it comes up within about 15 minutes and you can see that it did, the dose is generally repeated once. Later than that, assume it was absorbed and wait for the next scheduled interval rather than risk stacking. Repeated vomiting is itself a reason to call the doctor — and a reason to avoid ibuprofen.
Can I give acetaminophen and ibuprofen together?
They work by different mechanisms, so they are sometimes alternated for stubborn fevers — but only on a written schedule your paediatrician has approved. Running two clocks from memory at night is how interval errors happen. Never double up doses of the same active ingredient across products.
Should a higher temperature worry me more?
Not on its own.
